The IACUC
is implementing a new online protocol
submission system (myIACUC). The purpose of the
myIACUC system is to provide a more efficient way to prepare, submit,
approve, and track protocols through the IACUC protocol approval
process. We
encourage you to do all new or triennial submissions in the new
system now. The system will soon
be mandatory for all new or triennial submissions starting January
2, 2008. New and triennial forms will no longer be available on
this page and will not be accepted for IACUC protocol submissions
beginning January 2.

Animal Use Approval Request Forms
Various Animal Use
Approval Request Forms are available. Please use the most appropriate
form. The forms are updated periodically. Please make sure
you have the latest version of the form. It is important that all
sections are completed as described in the instructions for each item.
Proof-read your completed form. Incomplete, confusing or unformatted applications
will be returned without action.
Please
download the appropriate form from this page each time you submit a protocol
or modification thereof. Do not use old forms or "cut and paste"
from old forms.
Questions
and instructions do change occasionally, and corrections and clarifications
are made frequently.Double
clicking on these links will open the form for viewing only in
your browser. To download a form to your computer, Right Click
on the name of the document and choose "Save Target As" from
the menu that pops up. Save the form as a MS Word 2000 or XP file.
Forms in formats other
than MS Word will not be accepted for review. Remember that AUA Request
Forms are public regulatory documents. Please read all instructions
carefully, proof-read and spell check text before submitting the form
for review.
