Forms in formats other than MS Word will not be accepted for review. Remember that AUA Request Forms are public regulatory documents. Please read all instructions carefully, proof-read and spell check text before submitting the form for review.
REQUEST
FOR VISITORS TO DEMONSTRATE EQUIPMENT AND/OR HAVE ANIMAL CONTACT
If a company representative will be demonstrating equipment
or supplies, and will be having short term (less than five days) animal
contact, please have them complete this demonstrate equipment form
and submit it to iacuc@grove.ufl.edu.
It will be reviewed and approved by the IACUC chair.
For visitors to the University of Florida who will be handling animals, the IACUC requires that they assure the committee and the University that they have met their own institution's federal training requirements. Please see the requirements approved on June 27, 2008. A Visitor Certification Assurance form for visitors must be completed AND the signed ORIGINAL COPY returned to the IACUC office
If you have any questions about items on the forms, please contact the UF IACUC Office at 273-9535, or stop by the IACUC Office at the Broad Building, 1853 Mowry Road. This is on the NorthWest corner of Center Drive and Mowry Road. The main office is in room 125. The IACUC staff will be happy to assist you.
